Mechanism of action
Botulinum toxin type A is a presynaptic neuromuscular blocker. The active ingredient is the same in liquid and freeze-dried formulations — the difference is manufacturing format, not mechanism of action. Following injection into target muscle, the toxin’s heavy chain binds to specific receptors on motor nerve terminals; the light chain is internalised into the nerve cytoplasm, where it enzymatically cleaves SNAP-25 — a SNARE complex protein required for acetylcholine vesicle fusion at the neuromuscular junction. Without SNAP-25, the motor nerve cannot signal the muscle fibre to contract, and the targeted muscle relaxes. New axon terminals eventually sprout and restore neuromuscular signalling, at which point clinical effect wanes.
