Mechanism of action
Botulinum toxin type A is a presynaptic neuromuscular blocker. Following injection into target muscle, the toxin’s heavy chain binds to specific receptors on motor nerve terminals; the light chain is internalised into the nerve cytoplasm, where it enzymatically cleaves SNAP-25 — a SNARE complex protein required for acetylcholine vesicle fusion at the neuromuscular junction. Without SNAP-25, acetylcholine release into the synaptic cleft is blocked, the motor nerve cannot signal the muscle fibre to contract, and the targeted muscle relaxes. New axon terminals eventually sprout and restore neuromuscular signalling, at which point clinical effect wanes.
